YOUR ADVOCATE

Making decisions for yourself or for your loved one doesn't have to be difficult. We are your advocate! We offer flexibility in our plans and provide just the right amount of assistance (private duty, elderly, respite care) that allows continued independence based on each of our client's circumstances. 

YOUR RIGHTS

01/

Be fully informed in advance about services/care to be provided including the disciplines that furnish care and the frequency of visits as we as any modifications to the plan of care.

02/

Have your property and person treated with respect, consideration, and recognition of the patient’s dignity and individuality.​

03/

Be able to identify visiting staff members through proper identification.

YOUR RESPONSIBILITIES

01/

To provide complete and accurate information about illness, hospitalizations, medications, pain and other matters pertinent to your health; any changes in address, phone or insurance/payment information; and changes made to advance directives.

02/

To inform the organization when you will not be able to keep your home care appointment.

03/

If you need to cancel your services, we ask that you give the office 48 hours advance notice by calling (513) 326-5439.
